How Is Sleep Apnea Diagnosed in Malaysia?

Sleep apnea is usually diagnosed by combining a patient’s symptoms, physical examination, airway assessment and a sleep study. In Malaysia, patients with loud snoring, witnessed breathing pauses during sleep, choking or gasping at night, morning headache, daytime sleepiness or poor sleep quality may be advised to undergo further assessment for obstructive sleep apnea.

ENT airway assessment

For patients with snoring or suspected obstructive sleep apnea, ENT assessment may look for airway factors that contribute to poor airflow during sleep.

This may include examination of the nose, mouth, throat, tonsils, soft palate and neck. Nasal endoscopy may be performed when clinically needed to assess nasal blockage, deviated septum, turbinate swelling, nasal polyps, post-nasal drip or upper airway narrowing.

Sleep study test

A sleep study is the main test used to diagnose obstructive sleep apnea. It records breathing, oxygen level and sleep-related events during sleep.

Depending on the patient’s condition and available facility, the sleep study may be done as a home sleep apnea test or as an overnight polysomnography test in a sleep laboratory.

A polysomnogram is a sleep study used to assess sleep disorders, especially obstructive sleep apnea and snoring-related problems. Malaysian hospital sleep centres describe PSG as an overnight test that monitors sleep and breathing patterns.

Home sleep apnea test vs polysomnography

A home sleep apnea test is usually more convenient and may be suitable for selected adults with suspected obstructive sleep apnea. It is commonly used to measure breathing pattern, oxygen level, pulse rate and snoring-related parameters during sleep.

Polysomnography, also called PSG, is a more detailed overnight sleep study usually performed in a sleep laboratory. It can record more information, including sleep stages, breathing, oxygen level, heart rhythm, body movement and other sleep-related signals.

The choice of test depends on the patient’s symptoms, medical condition, risk level and the doctor’s recommendation.

Understanding AHI and sleep apnea severity

Sleep study results usually include an Apnea-Hypopnea Index, or AHI. AHI estimates how many breathing pauses or shallow breathing events occur per hour of sleep.

The doctor will interpret the sleep study result together with the patient’s symptoms, oxygen level, medical conditions and airway findings before advising treatment.

After diagnosis: what happens next?

Treatment depends on the severity of sleep apnea and the likely contributing factors. Options may include weight management, sleep position advice, treatment of nasal allergy or blocked nose, CPAP therapy, oral appliance discussion, or surgery in selected patients.

ENT assessment is useful when snoring or sleep apnea may be related to nasal blockage, enlarged tonsils, throat narrowing or other upper airway factors.
